Mouth Rehab Center (“we”, “us”, “our”) is committed to protecting your privacy. This policy explains how we collect, use, store and disclose information submitted through this website, in accordance with applicable United States privacy laws. This website is intended for dental practices interested in partnering with Mouth Rehab Center; it is not directed at patients or consumers, and no patient health information is collected here.
1. What information we collect
- Identity and contact details: your first name, practice name, practice website, email address and mobile number.
- Your responses to the partner application: information about your practice, including whether you have an in-house CBCT, the types of patients you can service, your monthly consultation capacity, and the payment plan and sedation options available at your clinic.
- Booking details: the day and time you choose for your partnership call.
- Technical and marketing data: the page you visited, the advertising campaign that referred you (e.g. UTM parameters and ad click identifiers), and anonymous usage events (such as which steps of the form were viewed). We use browser storage (localStorage/sessionStorage) to remember your progress and referral source. We do not use third-party advertising cookies on this site itself, though advertising platforms you arrived from (such as Meta) may set their own; see section 5.
By submitting your responses, you consent to us collecting the information described above for the purposes set out in this policy.
2. How we collect it
We collect information directly from you when you complete the partner application, enter your contact details, or book a call time. Technical data is collected automatically when you use the site.
3. Why we collect and use your information
- To assess your practice’s suitability for the Mouth Rehab Center partner network, including territory availability;
- To contact you by phone, SMS and email to arrange and confirm your partnership call and send reminders;
- To prepare for that call, including understanding your capacity and capabilities;
- To respond to your inquiries;
- To measure and improve our website, forms and advertising (including aggregate conversion statistics).
4. Who we share it with
We do not sell your personal information. We share it only with service providers who help us operate, under their own privacy and security obligations:
- CRM and scheduling: your details and application responses are stored in our customer relationship management and calendar system to manage your application and booking;
- Website hosting and infrastructure providers that serve this site;
- Communication providers used to send the emails and SMS described above.
5. Advertising platforms
If you arrived from an advertising platform (such as Meta), that platform may use its own cookies and identifiers, and we may share conversion events (for example, that an application was submitted) so the platform can measure and improve ad delivery. These events describe form milestones, not your individual application answers.
6. Data retention and security
We retain application information for as long as needed to evaluate and manage the partnership relationship, and as required by law. Information is transmitted over encrypted connections and stored with access controls.
7. Your rights
Depending on your state, you may have rights to access, correct or delete personal information we hold about you, and to opt out of marketing communications at any time. To exercise any of these, email contact@mouthrehabcenter.com and we will respond within a reasonable period.
